Reviews — pre-approval process (SoR)

IECL owns the pre-approval review as SoR (ADR-001 D1c): comments, checker step, rejection reasons. A CRITICAL comment that isn't resolved or dispositioned HARD-blocks the checker step and the release REVIEWS_CLEARED gate.
